What happens hour by hour on misoprostol day?
Before you take misoprostol, stock up on thick maxi pads and plan to stay home. Bleeding does not always start in the first hour, so a quiet start is not a sign that something went wrong.
Most patients begin to cramp and bleed within 1 to 4 hours. The flow can pick up quickly, and passing tissue or blood clots is a normal part of how the medicine works.

Through weeks 1 to 3, light bleeding or spotting is expected, and it can start and stop. That on and off pattern by itself is not an emergency. Watch the amount, which you can measure with pads, and watch how you feel, including dizziness, fever, or severe pain.
By weeks 4 to 6, spotting should be winding down for most patients, and your first normal period often comes back in that window. If you have no period by 6 weeks, call the clinic.

Are blood clots normal after the abortion pill?
Yes, and they are common. Passing clots, including some large ones, is part of how medication abortion empties the uterus. Clots on their own are not the warning sign.
The warning sign is how much you bleed over time. Go to the nearest emergency room if you soak through two or more thick pads in one hour for two or more hours in a row, even if you are also passing clots.
If you cannot tell whether you are seeing a lot of clots or too much blood, use the pad rule. Pads give you a number you can count, and worry does not.
Can I use tampons while I am bleeding?
No. Use pads only for at least 2 weeks after the abortion pill. Do not use tampons, menstrual cups, or anything else inserted into the vagina during that time, because they raise the risk of infection while your cervix is still healing.
Use thick maxi pads during the heavy hours, then switch to regular pads or liners as the bleeding gets lighter. Change your pad when it is soaked. Do not hold off just to count hours if you are already soaking through two pads an hour.

Go to the nearest emergency room if you have any of these:
- Heavy bleeding: soaking through two or more thick pads in one hour for two or more hours in a row.
- Severe pain: belly pain that ibuprofen and a heating pad do not relieve.
- Lasting fever: a temperature above 100.4°F for more than 24 hours after misoprostol.
- Foul-smelling discharge: this can be a sign of an infection that needs treatment.
